Service Description

Transitions between educational stages—such as starting school, moving from primary to secondary, or progressing into further education—are some of the most significant moments in a student’s learning journey. These changes can create excitement, but they can also lead to anxiety, uncertainty, and disruption to both academic progress and emotional wellbeing. Schools increasingly recognise the importance of structured transition strategies to ensure students feel supported, confident, and prepared. At JLW Consultancy, our Transition Support Service is designed to help schools build clear, effective, and student-centred transition processes that prioritise emotional wellbeing, curriculum continuity, and strong communication with families. Without a thoughtful transition plan, students may struggle to adapt to new routines, expectations, and environments, which can lead to disengagement, lowered confidence, and gaps in learning. Our service ensures that transitions are planned purposefully, supported compassionately, and aligned with the needs of every learner—including those with ALN/SEND. We work closely with schools to implement tailored, impactful transition practices, including: • Primary to Secondary Transition Planning that eases student anxieties, builds confidence, and prepares pupils for new routines and expectations. • Early Years to Primary Transition Support with developmentally appropriate approaches to help young children settle confidently into school life. • Post-16 Transition Guidance supporting further education choices, vocational pathways, and career planning. • Induction & Orientation Programmes including welcome days, buddy systems, and student-friendly resources. • Parental Engagement Workshops that empower families to support their child’s transition effectively. • Emotional Wellbeing & Resilience Sessions focused on managing stress, building self-regulation, and supporting anxious learners. • Academic Bridging & Curriculum Continuity ensuring smooth knowledge transfer and consistency in learning expectations. • Specialist ALN/SEND Transition Planning with personalised approaches, structured visits, and tailored support. • Cross-School Collaboration to ensure alignment, shared expectations, and effective communication between phases.
